LPA Applications have issued a Hints & Tips page for people to use when tackling Lasting Power of Attorney (LPA) applications. It has been claimed by the Office of the Public Guardian (OPG) that out of the staggering 67,000 forms sent every year, 15% have mistakes in them.

The 8 most common mistakes are:

1. Missing and mixing pages

2. Signing the application in the wrong order

3. Family members as certificate providers

4. Using initials instead of full names and not signing in the appropriate boxes

5. Pencil, Tippex and photocopies

6. Bound applications

7. Being unclear in the life sustaining treatment section

8. Contradictions in instructions and appointment types
